Purchasing additional perpetual name plates or name plates by mail:
The most cost effective way to order a single or several perpetual name plate(s) for a fantasy trophy or other perpetual trophy is to mail us a check for $7.50 for one name plate or $12.50 for two name plates.
Please be sure to include the following information:
- The text that you
would like us to personalize
- The size of the name plate (width and height) in inches
- A self-addressed first class stamped return envelope

- The fee for one name plate is $7.50 ($5 for personalization plus $2.50 for handling)
- The fee for two name plates is $12.50 ($5 each for personalization plus $2.50 for handling)
